Why All Initial Assessments Are Not the Same

The most valuable service we can offer our clients is to be able to understand their situation in order to create the most viable and sustainable solutions for their care needs.

In recent years, the term "assessment" has been used fairly loosely, oftentimes referring to an assessment by an outreach (not clinical) professional, a more medical-type of RN assessment that focuses primarily on physical needs or a "staffing" assessment, where the client defines his/her needs solely for the purpose of matching a caregiver.

The LivHOME assessment is different in these ways:

  1. You are dealing with an expert from the very first call: From a family's first contact with LivHOME, an expert in geriatric care will work to understand the senior's and family's needs, provide resources, and work with the family to recommend the best available options and resources for the complex situation. The initial phone consultation is complimentary.


  2. Clinical* assessment given by experts: LivHOME assessments are conducted by our professional Care Managers, credentialed experts in geriatric care with specialized training in social work, nursing or mental health, who work closely with each family to understand their specific situation. Based on a clinical assessment, the Care Manager develops a customized Plan of Care. The LivHOME Care Manager then works with the Caregiver and the family every step of the way to follow the plan, update it as needed, and ensure compliance.


  3. Holistic approach for a comprehensive plan: LivHOME not only looks at the immediate needs of providing a Caregiver, but takes a comprehensive approach to care that addresses the following needs:


    • Physical/medical (medications, home safety evaluation, cognitive/behavioral, fall risk, nutrition, etc.)

    • Socialization

    • Emotional

    • Planning and resources (legal, financial, insurance, entitlements, power of attorney, Medicare programs, etc.)
